Community Aged Care (CACPS) Packages
- Cobden and surrounding District (15 packages)- Cobden-Mortlake Area (10 Packages) (Alliance with Abbeyfield Society (Mortlake) Inc)
Community Aged Care Packages are planned and coordinated packages of care to help older members of the community remain in their own homes for as long as possible. They are funded by the Commonwealth Government.
Eligibility requirements for CACPs:
- 65 years and over and indigenous people 50 years and over- Have complex care needs
- Have strong desire to remain at home
- A need for a coordinated package
How can you access CACPs ?
To receive a CACP, you must be assessed as being eligible for the program. This is done by the Aged Care Assessment Service and will be done in the privacy of your own home.Your doctor can refer you to the Aged Care Assessment Service or you or your family can contact them for information on (03) 5561 9351.
